Case

  • 50 Years female pt came to casualty with history of thorn prick injury 15 days back.
  • She ignored the severity of wound.
  • She was not aware about her BLOOD GLUCOSE LEVEL.
  • Her Random Blood Glucose was 571mg% &HbA1C 11%.
  • Her foot wound increased .
  • On history her mother is also suffering from Diabetes since 10 years.
Result :
Now she has developed 3rd toe gangrene which needs amputation.

Case
Lipohypertrophy due to repeated insulin injections

  • This is localised Lipohypertrophy .Its due to repeated insulin injections at the same site.Patient feels less pain due to lipohypertrphy(Fat Collection).So they prefer the same site for insulin injections.
  • To avoid this patient has to rotate the injection site in the same area.Doctor has to ask for this type of swelling at injection site.This type of lipohypertrophy leads to increased insulin dose requirement as absorption is slow.
  • This type of lipohypertrophy is reversible



Case

57 years male patient with good Blood Glucose Level control developed severe sensorimotor polyneuropathy leading to toe ulcers due. He was using tight footwear which aggravated this problem.
Bilateral peripheral arterial pulsation are well felt.

It shows the importance of foot care n not only glycemic control.
